HOW TO ENTER:
• Entry forms will be available in the July 2021 Mililani News, downloaded at mililanitown.org or at the Rec 3 & Rec 7 Business Office starting on July 1, 2021.
• Member must fill out entry blank completely and DROP OFF at Rec 3 or Rec 7 Offices or MAIL IN to Admin. Office at:

DEADLINE: Entries must be RECEIVED BY 7 pm on Friday, July 23, 2021
RULES:
• Must be a MTA Member in good standing to enter.
• ENTRY LIMIT: ONE (1) entry per household / address. Duplicate addresses will be disqualified.
• MTA employees & members of their household are not eligible to enter.
• MTA Members can enter from July 1, 2021 – July 23, 2021.
• Entries must be RECEIVED BY 7 pm on Friday, July 23, 2021.
• Entry blanks with incomplete information will be disqualified.
• MTA is not responsible for lost or stolen entries, entries received after the deadline or illegible entries.
• Winners will be chosen by random on Monday, July 26, 2021.
• Once winners are verified, winners will be notified via email and/or phone by Wednesday, July 28, 2021.
• Winners can pick up prize at Rec 3 Business Office by appointment. Must provide ID and sign for prize.
• If prizes are not claimed by 7pm on Friday, August 13, 2021 an alternate winner will be selected for unclaimed prizes.
• Winners agree to have their full names printed in newsletter.
